Emergency Form

Your pet is being seen by our team of doctors and technicians on an emergency basis. Every effort will be made by our team to ensure that your pet is triaged and, if applicable, stabilized immediately upon arrival. 

If your pet is experiencing an acute crisis, is severely painful, or may be suffering a life-threatening injury, our team of doctors will immediately tend to them. If your pet is stable, they will be monitored in our hospital and examined by a doctor as quickly as possible. Our emergency patients are treated in order of the severity of injury, and our doctors will call you with updates once a diagnosis has been made and a treatment plan created. Additionally, you are welcome to call or text our office to check for any updates on a patient in our hospital. 

There is a $52.00 triaging fee for sick and critical patients who are being accommodated on an emergency basis in our hospital.

 
 
  • Once you have called our office to schedule your pet’s drop-off or emergency visit, complete the form below and click submit.

    Once you have submitted the form, please head to our office as soon as possible, so that our team can triage and, if needed, stabilize your pet.

    When you arrive at our practice, you may call to check in or head inside to check in at the front desk.

  • When your pet arrives, our team of technicians will take your pet’s vitals, and make sure that they are stable.

    If your pet is not stable, immediate measures will be taken to stabilize your pet. If your pet is stable and comfortable, they will await the first available doctor, who has worked them into their schedule.

  • When your pet is seen depends on how critical their illness is. Our team must prioritize critically sick and life-threatening patients first, before turning their full attention to patients who are stable.

    Your pet’s vitals and condition will be checked immediately upon arrival, so that any signs that they are in critical condition are not missed.

    If your pet is seen later in the day, it is because our team of doctors and technicians have evaluated your pet and determined that they are in more stable condition than other emergency patients.

  • A doctor will call you with any questions, as well as updates once diagnostics are performed and a treatment plan is made.
